One end of this old American pocket knife is squared off instead of rounded, and on some examples one of the two implements has no cutting edge at all. Neither one is a defect.

For decades a Pennsylvania cutlery company kept building a handle with a hard, flat corner on it, and the reason has almost nothing to do with cutting.

This is the story of the Case Doctor’s Knife, pattern 85: a slim, elegant slipjoint that most people today file under “gentleman’s knife,” and that was shaped around an American working routine which has since almost vanished. In 1930, roughly 40% of all patient-physician encounters in the United States happened inside the patient’s home. By 1980, that figure had fallen to about 0.6%. Between those two numbers sits the whole logic of this knife: the squared “pill crusher” bolster, the deliberately blunt spatula, a 1911 physician’s bag carrying a pocket medicine case of two rows of twelve bottles, and a division of labour between doctors and pharmacists that was still incomplete when this pattern’s ancestors were made.

We also untangle a detail that trips up a lot of collectors: pattern #85 versus the earlier #82 physician’s knife, and why the famous knife is not where the idea started.
